Kindergarten Registration

 

At the time of registration, parents will need to provide the following information:

1. A certified copy of the child's birth certificate. (Mother's copy cannot be accepted)

2. Tennessee Certificate of Immunizations. (Green Card)

3. Tennessee School Medical Examination. ( Green Card)

4. Proof of residence for the McKenzie Special School District. (Utility bill showing parent name and current address)
